In this initial meeting, we will review your health history and discuss your nutrition and wellness goals. Together, we will discuss evidence-based nutrition recommendations and formulate a personalized plan. This plan will be goal oriented and tailored to your unique health and wellness vision. 75 or 90 minutes
Follow-up Visit
We will review progress towards the goals set in your initial consultation. We will modify your personalized plan and set new goals as needed. 30 or 60 minutes

A complimentary 15 minute discovery call is recommended to determine if we might be a good fit to work together. Please contact the office to schedule.
Insurance
At this time, Nourished Nutrition Consulting is currently in network with Aetna, Horizon Blue Cross Blue Shield, and UnitedHealthcare. Medical Nutrition Therapy counseling sessions may be fully or partially paid for by insurance. It is recommended you verify your insurance benefits prior to your first session as your individual health plan may have specific requirements for coverage.
Patients residing in Arizona, California, Colorado, Connecticut, Michigan, New Jersey, and Virginia can receive counseling virtually via a HIPPA-compliant telehealth platform. Nourished Nutrition Consulting also provides in-person counseling services on Monday mornings in Northern Arizona.
Please use the link below to schedule your appointment using health insurance. After scheduling an initial visit, you will be contacted by Berry Street, a company that will verify your insurance benefits and provide you with initial paperwork.
Self Pay
If you are not in-network with the insurance plans above, or if your insurance does not fully cover Medical Nutrition Therapy, Nourished Nutrition Consulting offers cash-pay counseling sessions.
The fees are as follows:
75 minute initial consultation - $165
60 minute follow-up appointment - $130
30 minute follow-up appointment - $65
Our office can provide a superbill (a receipt of services) which you can submit to your insurance company for possible reimbursement. Please note, when submitting a superbill, there is no guarantee that your insurance will cover the costs.

Cancellation Policy
You are able to cancel or reschedule appointments prior to 24-hours before your original appointment time. Cancelling within 24-hours of your appointment or being more than 15 minutes late will result in a $100 fee charged to your payment method on file.
Good Faith Estimate
The Good Faith Estimate outlines an uninsured (or self-pay) individual’s expected charges for a scheduled or requested item or service by a healthcare provider. The Good Faith Estimate will also include items or services reasonably expected to be provided. If you are insured, you may also request a Good Faith Estimate of the amount you will be billed for services you might like to schedule.
